On your first visit to Charter Health Center, you will need to complete a Client Information form. The form contains two separate sheets: Client History and Client Information. Please arrive 15 minutes early to your first appointment to complete these forms. If you desire, you can download the entire form by clicking on the link below. This allows you opportunity to complete the questions before you arrive. Simply download the PDF file, print it out, fill it out, and bring it with you to your appointment.

You will need to have Adobe Reader installed on your computer to view and print the application form. A free version of the software can be downloaded from the Adobe's site.
